The answer to this question might beget other questions, but to start with who has an answer to this.
Does the three second in the lane count start when a player has one foot in the lane, or does he have to have both feet in the lane for the count to start.
Also, which referee is responsible for calling three second lane violations, or is it the responsibility of whoever sees it?

That is a great question. Let me take this, one step at a time.
The count starts when a player enters the lane with either or both feet. The count will stop when the player completely leaves the lane area,for a shot attempt,loss of possession, or a violation/foul.
Which official is responsible for the count depends on where the ball is when the player enters the lane. I would have to get into a long description of floor coverage to fully explain how that works. Once an official begins a count, he will stay with it until the opportunity to end the count or a violation occurs. One thing to remember, an official is aware of the lane and players entering in and leaving, but his concentration on that violation is secondary to so many other things going on around the basket.
